STARS Online Member Survey to Identify Pathways to Diagnosis

STARS Online Member Survey to Identify Pathways to Diagnosis

We are carrying out a survey of members to investigate the typical patient pathway to an accurate diagnosis of syncope.

Syncope (sin-co-pee), is a transient loss of consciousness caused by a sudden drop in blood flow to the brain, and usually results from a fall in blood pressure, or a very fast or very slow heart rate, or both.

We would be very grateful if you could take a few minutes to complete the following online questionnaire about your experiences. Your responses will remain anonymous unless you choose otherwise.

Is the survey relevant to you?

  • Have you been diagnosed with syncope?
  • Or, are you answering this survey on behalf of a relative (e.g. child / parent) or friend who has been diagnosed with syncope?

If your answers to the above questions are no, this survey is not relevant for you. If you are uncertain whether the survey is relevant to you or not, please call the STARS telephone help-line on 01789 450 564.

If you answered yes to either of the screening questions above, please fill out the questionnaire on the next page...
